After talking to the falling grave, then meet the NPC in the middle to gain the task of getting the 10 essences of fire, if you cancel the conversation with the NPC before choosing all of the options, you can't continue the instance. The torches and apostles of baphomet won't appear and the NPC in the center just keeps asking if you've obtained the essences of fire and the tablets. Then you must leave the instance, wait 4 minutes for the instance to destroy and start over.
