If, due to incosistent database, the same character (name, not CID) is the leader of one guild (zombie-guild?) and member of another, the guild in which he is a normal member will be deleted. It's because of the delete_char_sql code that checks guild table for leader name, not leader char id and, if succesful, deletes the guild that char is currently in, not the one from the fetched row...
