I don't know why, but I am using that configuration in my server and since the last maintenance, the HP bar is no updating (unless you change map, or things like that).
You can't either see the dot in the mini-map updated o.O

I cant reproduce, both the hp and the dots are updated when using party_hp_rate as 1
